Managing Health Care Decisions
We all face the possibility of being incapacity and unable to make or communicate our general wishes about our health care, or to make or communicate our decisions about treatment choices.
It is important to communicate your instructions about the intensity of treatment you desire, and about end of life issues to your health care providers ahead of time in case the time comes when you are not able to communicate those instructions personally. Advanced Directives or a living will give you the opportunity to communicate general instructions while you are still able to do so.
It is also important to designate someone you trust to make specific decisions about your health care if you are not able to make those decisions yourself. A durable power of attorney provides you with the opportunity to designate that health care decision maker.

You can use an Advanced Directive / Living Will to give instructions and guidance to your health care providers which are to be followed should you be unable to provide those instructions due to a terminal condition, persistent vegetative state, or end-state condition.

Durable Health care Power of Attorney
A Durable Health care Power of Attorney allows you to authorize someone you trust to make health care decisions for you if you are unable to make or communicate those decisions yourself. Your Durable Health care Power of Attorney can become effective at the time you sign it, or you can specify that it only becomes effective after your doctor certifies that you are not able to make your own health care decisions.
